This 59-year-old patient came to see me as she felt she was increasingly not looking like herself. She had always hated her chin and jawline particularly on the profile view. Dr De Silva asks his patients to bring an old photograph to look at how their individual face has changed with time and in this patient, we can see that she has lost a lot of natural volume. Although this is normal with time, restoring some of this natural volume loss will rejuvenate the face. Sometimes this can be done non-surgically with fillers however there has been an increasing trend to add too much or excessive fillers this often results in a puffy appearance which is highly undesirable as it no longer looks natural. Dr De Silva found that sometimes patients have had small amounts of filler added every six months to a year and sometimes patients do not realise how much filler has been added cumulatively over many years. Most facial ageing is a combination of some volume loss some effects of gravity and the skin becoming more lax with time. Key to a good jawline and chin in this patient was her underline bone structure, and we can see she has a small chin, this can be seen on the profile view as her chin sits several millimetres behind a vertical line drawn from her lip. In addition to rejuvenating a face with a vertical deep clean facelift Dr De Silva discussed with her options to insert a chin implant at the same time as her facelift. After a discussion regarding potential options for rejuvenating a face this lady underwent a vertical deep plane facelift and neck lift, with fat transfer, chin implant and temporal brow left with upper left plasty. I hope you agree this patient looks completely natural and really rejuvenated. Although key importance is lifting, of the neck requires platysmaplasty to contour the neck and jaw line.
Key to this improvement was looking at the individual ageing that had occurred in all aspects of her face. For example we can see there are increased hooding of lose skin in the upper eyelid that is creating a sagging appearance, by lifting the tail of her eyebrows and completing upper blepharoplasty her eyes look completely natural and rejuvenated. In the lower aspect of her face we can see multiple lines around her mouth and sagging sometimes these termed nasolabial and Marinette lines. We completed a vertical deep plain facelift with fat transfer and nano fat to improve this area. This is resulting in a completely natural rejuvenation by restoring a V shaped jawline and neck. By adding a customised chin implant at the same time as a facelift she now has a better profile than she ever had before. Most of Dr De Silva’s patients are looking for a completely natural rejuvenation, the balance in this is that although we want to improve lines and shadows, we do not want to remove every single line or every single shadow as this can look more plastic.
Although the surgery took over 7 ½ hours, Dr De Silva has developed a patented sedation anaesthesia formula avoiding the risk of general anaesthetic. This enables most patients to recover over about 30 minutes after and length the surgery and they are able to sit up to converse and eat soon after surgery. In De Silva’s opinion, sedation anaesthesia is at the forefront of science in terms of facial surgery as it provides patients a fast recovery with less side-effects from the traditional general anaesthetic.

This 61-year-old patient, came to see Dr De Silva having noticed a more tired appearance, particularly in her jawline and neck area, and in addition, tiredness around her eyelids associated with previous fillers, she had gone undergone a previous facelift procedure and eyelid procedure some years before, with some improvement. However, the results had not lasted. Dr De Silva noticed this puffy appearance around the lower eyelids, which is often a consequence of fillers in this area. Although fillers can be helpful to reduce facial ageing, the lower eyelid area is extremely delicate, and some days, Dr De Silva has dissolved more than three patients’ fillers in a single day. This area of the lower eyelids is much more sensitive than other parts of the face, and a combination of the wrong type of filler (Hydrophilic = water loving) in the wrong place and the genetics of a person can sometimes mean that fillers in this area do not dissolve after 12 months, but can persist for even up to 10 years. Each year, they can get more and more puffy, resulting in an unnatural appearance. The first step Dr De Silva takes is to dissolve the filler with a product called hyaluronidase. It is important when dissolving filler in the lower eyelid area that there is a plan to correct this with fat transfer and lower blepharoplasty within one to two weeks of dissolving the filler to avoid the patient being left with a sunken and hollow appearance. This patient underwent Dr De Silva’s customised vertical deep plane facelift and neck lift with lower blepharoplasty and fat transfer under sedation anaesthesia.

Although not all patients want this level of detail, it is available to patients should they see fit. As important as the surgery is management of patients’ recovery, Dr De Silva uses a combination of techniques, to enhance and speed up recovery including hyperbaric oxygen, regenerative medicine and a type of lymphatic drainage to help reduce swelling faster after surgery. Dr De Silva’s found that swelling is the main factor in healing after surgery, and by taking measures to reduce that swelling faster, patients are able to get back to their normal social and professional activities as soon as possible. Lower eyelid bags in this younger person was a result of the anatomical position of his eyelids in a relatively forward position in relation to his cheek, referred to as negative vector. Dr De Silva performed trans-conjunctival (hidden incision) lower blepharoplasty with canthopexy (lower eyelid support). In addition the man had sustained an fracture of the floor of his left orbit that had resulted in mild asymmetry and scarring. Important considerations in this case is that in a young person, some fat should be transposed and preserved to avoid hollowness, conservative surgery is advised to give patients natural looking results. The ‘after’ photograph shows the appearance of the eyelids at 5-weeks after surgery and shows an absence of fat bags and a smoothened contour between his eyelid and cheek associated with youth.
