Lower blepharoplasty is a specialist eyelid surgery that rejuvenates the under-eye area by addressing puffiness, eye bags, and hollowing. These concerns are typically caused by aging, genetics, or lifestyle factors and make you appear tired or older than you feel.
This procedure targets the fat pads beneath the eyes, which can become more prominent or shift with age. Lower eye surgery restores a smooth, refreshed contour by repositioning or conservatively removing this fat.
Dr De Silva uses refined, scar-concealing techniques to preserve the natural shape of your eyes while offering long-lasting improvement. For many, the results are transformative, giving a more rested, youthful appearance that creams or fillers simply can’t achieve.

Many patients have tried creams, lotions, or non-surgical fillers without success. Fillers in the lower eyelid can be unpredictable due to the delicate nature of the skin and often lead to puffiness as they absorb water over time. Dr De Silva frequently dissolves poorly placed fillers before correcting the area surgically with techniques such as lower blepharoplasty, fat transfer, laser treatment, canthopexy, and skin pinch.
"Some weeks, I have dissolved filler for three or more patients and corrected it surgically. Often, it’s not just about removing fat, it’s about restoring natural contours that last for years," says Dr De Silva.

While both procedures aim to rejuvenate the eye area, they address very different concerns. Upper eyelid surgery focuses on removing excess skin that causes heaviness or drooping, often improving both appearance and vision. In contrast, lower eye surgery targets puffiness, bags, and hollowing beneath the eyes, concerns that topical treatments and fillers rarely resolve long-term.
Lower blepharoplasty eyelid surgery requires more delicate handling of intricate anatomy to avoid complications and preserve natural eye shape. The techniques used are more specialised, often involving fat repositioning and internal incisions to achieve a refreshed look without visible scarring. Dr De Silva’s expertise ensures that each approach is customised, delivering balanced, harmonious results whether performed alone or alongside upper blepharoplasty.
Dr De Silva specialises in facial procedures only, with blepharoplasty being the number one surgery he performs. In most cases, he uses a transconjunctival approach to lower blepharoplasty eyelid surgery, placing the incision inside the lower eyelid to avoid visible scarring. Though technically more complex, this technique offers a faster recovery and a natural result.
The lower eyelid contains intricate anatomy that must be preserved to avoid complications such as eyelid retraction. Dr De Silva uses CO₂ laser or monopolar electrocautery for precise incisions and immediate control of bleeding, reducing swelling and speeding up recovery.
The fibrous orbital septum is carefully opened to access the fat pockets responsible for under-eye bags. Instead of simply removing the fat, which can result in a hollow or aged appearance, Dr De Silva uses a conservative, tissue-sparing approach. He typically repositions the fat (fat transposition) into the tear trough to restore a smooth, youthful contour. In some cases, a combination of repositioning and removal is used.
A dissolvable canthopexy stitch is sometimes placed to support the lower eyelid during healing, improving both the result and eye shape. Older surgical techniques involved incisions below the lash line, often resulting in visible scars and eyelid distortion. These are now largely avoided in favour of more advanced approaches.

I would like to share my experience of my face and neck lift with Dr De Silva. First and foremost I will tell you truthfully, that with all I now know for my experience, I would go through it all again tomorrow if I had too. It was so different from what you hear, and all the botched jobs you see. I picked Dr De Silva after much research, and the fact that he only specialises on the face, I made my appointment. From my first consultation till my 1yr check up, I can only but give him and his team 100% Dr De Silva has an extremely calming way with him. He puts you at your ease and explains EVERYTHING in minuet detail. He leaves nothing out, everything he said happened in that way. From having pre op’s till the day of surgery nurses Ellie and Josie were fantastic, so helpful and friendly. I was told by Dr De Silva that It would not be painful, just a bit uncomfortable, that was true. I had no pain at all at anytime from start till now, Swollen and bruised yes, the bruising had all gone in 2 weeks. Swelling took a little longer. I was also told it would be quite numb for some time. It was, but it didn’t bother me, and now I have very little numbness left. It can take up to 9 months I think. I didn’t want to look different, I didn’t want to change anything, all I wanted was to look rested and fresher, a better version of myself. I am soooo happy with my lovely tight neck and no more loose skin and hanging jowls. It has made the biggest difference to my confidence. No one has asked have I had work done, they just say “ you are looking great”. If you are thinking of a face lift or any facial work done, I can 100% guarantee you will not find a better surgeon… anywhere. My extreme gratitude to Dr De Silva, Dr Andre (anaesthetist). Nurse’s Ellie & Josie and to Sarah who sorted all the paperwork etc.
Lower blepharoplasty eyelid surgery is ideal for individuals bothered by puffiness or hollowness under the eyes.
Thanks to Dr De Silva’s advanced use of laser technology and transconjunctival lower blepharoplasty techniques, recovery after lower blepharoplasty eyelid surgery is typically smooth and manageable. Most patients report only mild discomfort and are able to see clearly immediately after the procedure. You’ll return home within an hour, without the need for an overnight stay or general anaesthetic.
A dissolvable stitch may be placed to support the eyelid as it heals, enhancing both comfort and aesthetic outcome. Bruising and swelling are usually minimal and begin to settle within the first few days. Most people feel confident returning to daily activities in 7–10 days. Final results continue to refine over the following weeks and months, with scars, if any, discreetly hidden and virtually undetectable.
Although lower eye surgery is generally safe in expert hands, all surgery carries some risks. Dr De Silva minimises these risks through meticulous technique and advanced technology.
